Introduction
Welcome to the Organizational Assessment for Maternal Health Integrated Care Model (MO C3).
MO C3 is dedicated to supporting sites in creating a Maternal Health Integrated Care Model that is flexible and guided by agreed-upon core elements. This assessment serves as a vital first step in understanding your organization's current model, capacity and identifying areas for growth in aligning with these core elements. The insights gathered will directly inform our work together, including tailored technical assistance and resources provided by MO C3 as we work to develop a sustainable care model.
Guidance for Completion and Best Practices
Goal: The primary goal of this assessment is to capture a holistic and accurate picture of your organization's current practices and infrastructure as they relate to integrated maternal health care.
We ask that this assessment be completed collaboratively as a team rather than by a single individual. Below are some suggestions for assessment completion:
- Form a Core Assessment Team – include as many team members as possible
- Schedule a Dedicated Working Session(s) – block off uninterrupted time for the team to review & complete the assessment
- Designate a Facilitator & Secretary – the Facilitator guides the discussion while the Secretary is responsible for digitally completing the responses
Time Commitment: We estimate that completing this comprehensive assessment will require approximately 90 minutes of dedicated time for your core team.
Submission Deadline: All completed Organizational Assessments must be submitted no later than May 15th.
Instructions
- Focus on Current State: Please answer each question based on your organization's current operational reality, not on planned or aspirational future states.
- Confidentiality: Your input is invaluable and confidential. It will be used solely for the purpose of guiding MO C3 support and technical assistance for your organization.
- Saving your Progress: Upon saving your progress, you will receive a link via email that will allow you to resume your assessment from where you left off. To navigate back to the main section from the Assessment, use the MO C3 button on the left-hand menu.
- Completion: Ensure all sections are fully completed to the best of your ability before final submission
